چکیده

Introduction The preferential origin of atrial tachycardia (AT) is generally associated with specific anatomical atrial structures such as crista terminalis, tricuspid annulus, mitral annulus, and pulmonary veins. There are 3 major mechanisms of focal AT: enhanced automaticity, triggered activity, and microreentry. Focal AT can be eliminated via radiofrequency ablation with fairly good long-term outcomes. In previous reports, atrial fibrillation (AF) with pseudo–atrial flutter had been presented; in those cases, the organized flutter rhythms were mostly confined far from AF origins. In our cases, we successfully entrained the narrow complex tachycardia. Fractionated potentials were present in the electrogram at the earliest tachycardia activation localized to a small area (0.25 cm), which suggested a mechanism of focal AF. Radiofrequency ablation at the earliest activation sites with fractionated potentials terminated the focal AF and AT. The objective of this case report was to demonstrate the underlying mechanisms of AT as a manifestation of focal AF.

Successful cryoablation of an incessant atrial tachycardia arising from the right atrial appendage.

The right atrial appendage can be the origin of focal atrial tachycardias. Their ablation can be challenging owing to the complexity of the appendage anatomy. To our knowledge, we describe the first successful solid tip cryoablation of a focal tachycardia within the right atrial appendage in a patient presenting with tachycardia-induced cardiomyopathy.

Intravenous Methylprednisolone, a Possible Cause of the Atrial Fibrillation

We are presenting a case illustrating atrial fibrillation (AF) following the use of methylprednisolone in a patient with pelvic and femur fracture. A 48- year- old man with no significant past medical history, was admitted to the emergency department after injury in a car accident. He suffered a multiple bone fracture with chief complaints of pain and shortness of breath. He was transfer...

Ablation of focal atrial arrhythmia in patients with congenital heart defects after surgery: role of circumscribed areas with heterogeneous conduction.

BACKGROUND In patients late after surgical repair of congenital heart disease (CHD), areas with abnormal electrophysiologic properties may serve as slow conducting pathways within a macroreentrant circuit or may be the source of focal atrial tachycardia. OBJECTIVES The purpose of this study was to evaluate the role of abnormal areas during focal atrial tachycardia prior to ablation. METHODS...

Arrhythmias in Patients With Pulmonary Hypertension; A Narrative Review of the Current Literature

Incidence and prognostic relevance of supraventricular arrhythmias were evaluated in nine studies. These studies investigated patients with different forms of pulmonary hypertension. Supraventricular arrhythmias were more often found in patients with group two pulmonary hypertension. Common findings in these patients were elevated right atrial pressure and diameters and reduced tricuspid annula...

Adenosine and Atrial Tachycardia

Atrial tachycardia (AT) may be encountered in a variety of settings in the electrophysiology laboratory. It constitutes a differential diagnosis in patients presenting for an electrophysiological evaluation and ablation for paroxysmal supraventricular tachycardia. These patients typically do not have any evidence of structural heart disease. Patients may also present with AT in setting of an at...
